Before your first visit

Please arrive about fifteen minutes early for your first appointment so we can complete intake paperwork. Bring:

  • Photo identification
  • Your insurance card
  • A list of any current medications
  • Contact information for other providers involved in your care

If you need to reschedule, please give us 24 hours' notice when you can so we can offer the slot to someone else.

Growing across Michigan

We are working toward opening a second NorthStar location in Grand Rapids, and we have an agreement in progress with a smaller practice in Gratiot County that we expect to complete around the end of the year. Both will operate under the same clinical model and leadership as our Detroit office.
