How we protect your information, and what you are entitled to.

Notice of Privacy Practices

Effective date: January 1, 2024

This notice describes how medical information about you may be used and disclosed, and how you can get access to this information. Please review it carefully.

Our commitment

NorthStar Behavioral Health & Recovery is required by law to maintain the privacy of your protected health information, to provide you with this notice of our legal duties and privacy practices, and to notify you following a breach of unsecured protected health information.

Substance use records receive additional protection

If you receive substance use disorder treatment from us, those records are protected by federal regulations at 42 CFR Part 2 in addition to HIPAA. In general, we may not tell anyone outside our program that you attend, or disclose information identifying you as having a substance use disorder, without your written consent — except in the limited circumstances the regulations allow, such as a medical emergency, a court order, or a report of suspected child abuse or neglect.

How we may use and disclose your information

We use and disclose health information for treatment, to obtain payment for services, and for our own health care operations such as quality review and staff training. We may also use or disclose information where required by law, for public health activities, for health oversight, and to avert a serious threat to health or safety.

Your rights

To exercise any of these rights, contact our Compliance Manager at compliance@northstarbhr.com or by calling our main office. You may also file a complaint with the U.S. Department of Health and Human Services Office for Civil Rights. We will not retaliate against you for filing a complaint.

We provide free aids and services to people with disabilities and free language assistance services to people whose primary language is not English. To request an accommodation or an interpreter, please tell us when scheduling your appointment.

Changes to this notice

We reserve the right to change this notice and to make the revised notice effective for health information we already have as well as any information we receive in the future. The current notice will always be posted on this page and available at our office.
